While in eighth grade, Garland joined the starting lineup for the Brentwood Academy varsity basketball team. Among division II-AA players, he was one of the three finalists for Tennessee Mr. Basketball. As a sophomore, he claimed most valuable player (MVP) honors at the King of the Bluegrass Holiday Classic on December 23, 2015. Per game, he averaged 18.6 points and 4.3 assists and earned TSSAA Division II-AA. In its Sophomore All-American Team honorable mention, high school sports website MaxPreps included him. By his fourth season with Brentwood Academy, he was ranked the best point guard in the 2018 class.
Garland garnered Naismith Trophy High School All-America honorable mention recognition in March 2017. Furthermore, the extremely talented player led his team to a 30-2 record and state championship. The 2018-19 season was entered by Garland as the best recruit to ever sign with Vanderbilt. In fact, he was the only freshman in the Southeastern Conference (SEC) to make the preseason all-conference team. During a layup versus Kent State on November 23, he injured his knee and left the game early. It was revealed by the Vanderbilt head coach four days later that he would be sidelined for the remainder of the season. Garland disclosed that he would leave Vanderbilt to recover from his injury and prepare for the 2019 NBA draft on January 22, 2019. Later, he became one of 77 total invitees to take part in the NBA Draft Combine. With the notion that he was promised selection by a team in the NBA draft lottery, he left early on May 15.

With the fifth pick by the Cleveland Cavaliers in the 2019 NBA draft, Garland was selected. Cleveland Cavaliers announced that they had signed with Garland on July 3, 2019. Garland made his debut in NBA on October 23, 2019. During this game, he made history by becoming the first player born in the 2000s to make his NBA debut. He was selected to his first NBA All-Star Game as a reserve on February 3. In voting for the Most Improved Player award, Garland finished third behind Ja Morant and Dejounte Murray.
